Atlantic Health System is seeking a Per diem Social Worker, within the Atlantic Homecare & Hospice division.

Essential Functions/Responsibilities of Position:

• Home visits with home care patients, providing disease counseling, end of life discussions with goals of care, community resource referrals, assistance with applications to community resources, Bi-lingual a plus

Qualifications:

Minimum Education:

• Master's in Social Work from an accredited school of social work required

License/Certifications:

• NJ Social Work License LSW or LCSW required

Relevant Experience:

• Disease counseling and community resources
