POSITION OVERVIEW: The Medical Group Primary Care Clinic Lead Physician, in a dyad relationship with the Clinic Manager, provides operational leadership for the Primary Care Clinic. ("Clinic") . Together with the Clinic Manager, the Clinic Lead Physician is responsible for the achievement of Primary Care Clinical Division strategies by the Clinics, and management of Clinic operations to achieve St. Charles Health System's True North objectives. The Clinic Lead Physician, in collaboration with the Clinic Manager, is also accountable for operating the Clinics in compliance with all applicable policies of St. Charles Health System and its medical staffs. The Clinic Lead Physician participates as a member of the SCMG Primary Care Leadership Council, and also as a member of an MGB Committee or SCMG Management Committee, as requested.

ESSENTIAL FUNCTIONS AND DUTIES: (May perform additional duties of similar complexity within SCHS as required or assigned)
Dyad Relationship: As the physician leader in the dyad, the Lead Physician is primarily responsible for the quality of clinical professionals and of the clinical work in the Clinics. The Lead Physician is responsible for ensuring that Clinics' physicians and advanced practice professionals maintain core Professional Competencies, including: Practice-Based Learning and Improvement; Patient Care and Procedural Skills; Systems-Based Practice; Medical Knowledge; Interpersonal and Communication Skills; and Professionalism.
The Lead Physician is also accountable for physician and team productivity; for clinical innovation; for compliance; for development, implementation, evaluation, and improvement of clinical pathways and models of care; and for relationships with referring and consulting physicians.
The Lead Physician is responsible for supporting the Clinic Manager in the dyad roles, including the provision of administrative services; revenue management; operating expense management; capital planning and allocation; development, implementation and evaluation of staffing models; operations performance reporting; supply chain management; and support systems services.
The Lead Physician also provides clinical medical care to patients of SCMG.

Expand access by wider application of telemedicine and remote monitoring technologies
Plan, design, develop and implement advanced team-based care models to improve care delivery in partnership with members of the St. Charles care team, including, as appropriate, referring physicians, palliative care, Home Health & Hospice care, care coordination, and patients and families
Advance the roles of patients as partners in care through systematic implementation of Shared Decision-making Models
Systematically implement standards of clinical practice that achieve or exceed the standards for processes of care of the major professional medical and nursing societies appropriate for the Division and Clinics

Achieve top percentile ranking on surveys of caregiver engagement and resilience
Ensure that every caregiver is empowered as a leader and as a team member through robust and systematic deployment of the principles of workforce inclusion and diversity
Give voice to caregivers by facilitating and coaching at regularly-scheduled meetings of the Clinics' team
Conduct annual physician and advanced practice professionals performance reviews
In partnership with the Clinic Manager, provide a role-model for teamwork and leadership
With support from the SCMG Office of Professional Services and the Clinical Division, participate in the Division's Physician and Professional Work Force Development planning so that
Each physician and advanced practice professional has a current career development plan, and
The Division retains and develops an excellent clinical work force to meet community needs
Oversee Ongoing Professional Practice Evaluation and Focused Professional Practice Evaluation for each member of the Clinic

Participate actively in the Clinical Division's Patient Safety Program, founded on the principles of patient safety science.
Implement a culture of safety at the Clinics to ensure vigilance in patient care.

In collaboration with the SCMG Committee on Quality Innovations and the Clinical Division, implement evidence-based, redesigned care paths to eliminate the internal and external failure costs of poor quality.
In collaboration with the SCHS Population Health team, implement evidence-based models for addressing social determinants of health in the care of patients
Optimize processes for addressing financial barriers to care
Develop Social Service metrics for timely enrollment in OHP for eligible patients
Support Social Service identification of financial barriers

In collaboration with the SCMG Committee on Quality Innovations, implement evidence-based, redesigned care paths to eliminate the internal and external failure costs of poor quality.
Measure and report care outcomes, and benchmark against best practices
Achieve the standards of care at the Clinics as established by the Clinical Division
Participate as a leader in accreditation surveys and programs, as requested
Participates in creating a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver.

Graduation from an LCME accredited medical school.
Successful completion of an ACGME accredited residency program in Family Medicine or Internal Medicine
Oregon license as MD/DO
Board certification by the American Board of Family Medicine or the American Board of Internal Medicine
Demonstrated leadership experience in integrated delivery system medical group or faculty practice departments.
Valid driver's license and the ability to meet SCHS driving requirements.
Maintain unrestricted DEA number.
Maintain unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.
Epic EMR software experience

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

Intermediate to advanced proficiency with Electronic Medical Record systems.
