The Community Nursing Home (CNH) Community-Adult Day Health Care (C-ADHC) RN is responsible and accountable for providing clinical oversight of Veteran care in the CNH Program primarily, and C-ADHC program as assigned. Major duties and responsibilities include, but are not limited to: Practices a high degree of leadership, motivation, self-direction, and initiative in quality management activities to improve care. Broadly acquainted with clinical treatment modalities, administrative and clinical support systems and knowledgeable about the VA multifaceted health care delivery systems and relationship to clinical practice guideline development and implementation. Demonstrated clinical competencies in such areas as age-specific care, cultural assessment, and health education. An ability to effectively function autonomously, as well as a member of an interdisciplinary team as needed for family meetings with SNF's, A commitment to discipline-specific standards of practice, including ethical conduct, the primary care delivery model, chronic disease management, palliative care, principles of care management, a holistic framework of practice, and geriatric care skills (including infection control, equipment and environmental safety). Monitor Veteran for new onset and/or worsening skin integrity Determine when pain is assessed with appropriate interventions and outcome Monitor for fall risk at facility/home environment, safety hazards, medication errors, bowel regime, AD/IADL functionality.
