The LPN Community Living Center (CLC) is located at the Roseburg Healthcare System. The LPN CLC services Veterans requiring long term care, dementia care, palliative care, hospice care, rehabilitation, skilled nursing, respite, and geriatric evaluation and management. The CLC serves as a responsible member of the nursing team and appropriately interacts with Veterans, family members, clinicians, and supportive personnel related to patient care activities and outcomes. Duties include but are not limited to the following: The work involves the performance of a moderate range of functions using guides, policies, and direct or adjacent oversight by a more experienced LPN, RN, or LIP. May work under the direction of a senior LPN 7. The work involves a variety of patients of diverse cultural, spiritual, and education backgrounds. The work directly affects the overall well-being of patients served. The work involves scheduled appointments following accepted facility practices. Recognizes appropriate materials and techniques used with each patient and develop the ability to recognize less familiar and frequent medical care needs and apply previously applied guidelines to the situation. Recognizes that illness and/or hospitalization may cause fear and anxiety which patients may express in a variety of behaviors and is able to respond in an appropriate manner to those patients. Identifies age-specific or population-specific learning needs and provides patient education materials and documents teaching in the medical record. Promotes team cohesion, participates in unit activities to enhance positive work environment, and contributes to performance improvement activities. Collect data regarding a client's status, record objective and subjective data in an accurate and timely manner and report the data in a timely manner to the RN to use in completing a nursing assessment of the client. Identifies physical and emotional changes in patient condition. Demonstrates practical knowledge of human behavior, motivation, and reactions to situations.
