Salary for this position ranges from $55K-$70K/year based off experience
You will travel throughout Madison County and surrounding areas to care for our home health patients
This is a full-time position with on-call/weekend rotation
The Home Health Care Manager provides in-home medical care and living assistance to our patients. The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing. The focus of the Home Health team is to help patients return to their normal routine.
Active Registered Nurse license in the State of Ohio without any board actions.
Two years acute care nursing experience preferred; Previous Home Health experience is a plus.
Computer skills sufficient to properly document services and care.
Ability to drive during daytime, nighttime or inclement weather.
Valid Driver's License with Safe Driving Record
State Minimum Automobile Insurance Coverage
