The West Palm Beach VA Healthcare System is seeking an experienced full-time Physician Assistant or Nurse Practitioner with Geriatrics and Extended Care, Community Living Center and Home-Based Primary Care. Major duties include, but are not limited to: Assist in evaluation and treatment of patients with Congestive Heart Failure (CHF). Evaluate and apply for transplant/left ventricular assist devices (LVADs) for patients with advanced heart failure. In an outpatient setting diagnosis, determine cause and precipitating factors to CHF, recommend and implementation of GDMT (guide-line directed medical therapy), provide education to patient and family , follow up post hospital discharge, reconcile medications and answer questions, and return phone calls by patients. Arrange for outpatient monitoring via Telehealth if needed or other ancillary service. Return calls from outpatient, and sometimes manage volume overload before admitting to hospital. Treat and monitor patients that have advanced therapies with mechanical circulatory device (like LVADs), home inotropes, heart transplant, or refer to Hospice. Educating patients and family regarding all steps in transplant/LVAD process. Coordinate care of LVAD/transplant patients with Transplant Center (i.e VAMC or TGH), write prescriptions, adjust medications, including transplant medications under supervision of Cardiologist. Work Schedule: Varying shifts and tours based on the needs of the service, may include holidays and weekends Financial Disclosure Report: Not required Nurse Practitioner Salary Range: $70,323 - $129,462 Physician Assistant Salary Range: $97,989 - $127,389 This Physician Assistant (not Nurse Practitioner) position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program within VHA. If selected for this position, you are encouraged to apply for this award program and must meet specific individual eligibility criteria in accordance with VA Policy 1021. Employees must apply for EDRP within four (4) months of appointment. Final award amount and program start date is determined after review of a complete EDRP application and supporting documentation by the VHA Education Loan Repayment Services team. Maximum award is total amount of qualifying loans up to $200,000. If selected, the local HR POC will provide the EDRP contact information for application materials and procedures.
