San Francisco VAMC is a major interdisciplinary teaching facility with service-lines serving medical, surgical, and neurological inpatients and outpatients, with a Level III Emergency Department, and multiple community-based outpatient clinics (CBOC). There is a large outpatient psychiatric population, with admissions for medical comorbidities. Home-Based Primary Care is provided in the veteran's home. The Nurse Practitioner has leadership in delivering and improving holistic care through collaborative strategies with others. S/he has the ability to manage complex patient situations. S/he is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. S/he assesses, plans, implements and evaluates care based on age-specific components. S/he considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences in the provision of care. S/he prescribes and administers medications and procedures per established policies and guidelines. S/he influences care outcomes by collaborating with members of the interdisciplinary team. S/he assumes leadership responsibilities at the patient and Teamlet levels. Core elements of performance include ability to recognize and respond appropriately to medical, knowledge and active participation in unit/program level quality improvement processes, and initiatives as well as customer service programs. Care is provided within the Clearlake CBOC addressing preventive health care, health maintenance needs and primary care to eligible veterans. Also, same day health care services and other integrative services such as nursing, pharmacy, social work, nutrition and behavioral health are offered in this setting. The NP may be assigned as a back-up provider who is responsible for providing primary care to the patients of providers who are unavailable in order to offer full primary care services in the absence of the PCP. The back-up NP also acts as surrogate to cover for provider absences for view alerts, secure messages and other needs for the absent provider's panel.
