As a Home Care LPN with the Senior Services Department of Fairview Home Care and Hospice, you would be working with geriatric patients to provide skilled nursing care according to the plan of care in home environments. Most visits would be within Assisted Living Communities, Memory Cares, and Independent Senior Housing buildings, and occasionally to private homes.Provides services under the supervision of a qualified registered nurse. Reports pertinent observations to the Case Manager/RN.
This position does require traveling to several facilities within a geographical radiusin Minneapolis and surrounding suburbs and is not a position located within one facility.

Consults with the Nurse Case Manager to make modifications in the plan of care and obtains physician orders as delegated by the RNfor all changes made in the plan of care.
Counsels the home care patient, family and caregiver regarding care needs and health promotion/prevention and reinforce teaching/education at the direction of RN.
Monitors the patients progress and responses to care/services delivered, and update the Nurse Case Manager, physician and other health care team members as indicated.
Maintains complete, accurate and timely clinical records/documentation on all patients according to agency policy.
Stays current (expert) in home care regulation and all applicable licensing.
Actively participates in educational programs, meetings and agency committees as appropriate.
Accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures.
Identifies processes to improve and potential solutions.
Licensed as an LPN and eligible to practice in Minnesota
Current CPR (BLS for Health Care Providers) certification
Six months recent experience in a med/surg, long term care, or home care/hospice setting as a CNA, HHA or LPN.
Two or more years recent LPN experience in a med/surg or home care/hospice setting.
Valid Minnesota drivers license (or license that matches your current permanent residence)
Current and adequate automobile insurance policy
Computer skills (able to document effectively in EMR)
