Fairview Home Infusion is a nonprofit provider with over 25 years of experience, offering personalized home infusion care for adults and children of all ages. We care for patients across the five state areas of Minnesota, Wisconsin, Iowa, North Dakota and South Dakota.
Fairview Home Infusion staff provides one-on-one home infusion nursing care visits and patient education in accordance with the physicians plan of treatment. Responsibilities include teaching patients about their home infusion therapy, administering IV therapies and providing ongoing monitoring of their treatment. Fairview Home Infusion seeks RNs who are customer-service oriented and seeking growth.
This position is a casual day position based out of the Minneapolis facility.
The Clinical Support LPN performs and assists in the performance of general office duties, computer operations including data entry, reporting, and correspondence, as well as maintains an efficient, effective, functional and organized office environment.
Foster and create the relationships between staff and clients.
Document and communicate clearly and effectively over the phone, in person, and in writing with clients, family, caregivers, case managers, and other members of the home health care team.
Organizes and prepares work schedules for nurses and home infusion patients.
Enter and maintain schedules in database, and monitor client and staff hours to avoid unnecessary overages.
Provide phone and triage assistance.
Minimum 2 years recent experience as an LPN
Computer skills: able to document effectively in EMR and experience with Windows based applications
Current LPN license in the State of MN
Current CPR certification (BLS for Health Care Providers)
Previous customer service experience, Home Care and Hospice experience, skills related to medication and dosage.
