Registered Nurse responsible for total nursing case management of patients enrolled in specialty case management programs. Responsibilities include, but are not limited to, clinical intake, lab coordination, act as liaison with physicians and/or other clinicians involved in patient care, and ongoing patient follow up to assure compliance with therapy and effectiveness of treatment.
Provides efficient, accurate and safe home infusion nursing case management services in a manner consistent with high quality standards of a professional registered nurse and in compliance with accrediting bodies. Meets the qualifications to assess, create patient specific plans of care and educate patients of a broad age range about their home pharmaceutical therapies and devices/equipment needed to administer medications. Acts as a liaison to home health agencies to address performance and staff competency issues, coordination and planning of care, continuing education and other operational issues. Performs case management and consulting functions to the health care team and reimbursement staff in a managed care setting. Understands and anticipates problems associated with home therapies and utilizes safe problem solving skills. Participates in public relation activities.
Performs duties to support the goals and objectives of the Geisinger Health System in its efforts to provide quality patient care in a cost effective manner, achieving a high level of patient satisfaction.
Works under the general supervision of the Regional Coordinator of Ambulatory Clinical Pharmacy Programs.
SKILLS AND ABILITIES:
Demonstrates a broad clinical knowledge base and proficient clinical assessment skills to function as an infusion nurse specialist.
Demonstrates knowledge of basic concepts and principles of managed care, infusion therapy and vascular access devices.
Demonstrates general computer knowledge and capability to use computers.
Demonstrates customer-focused interpersonal skills to interact in an effective manner with the practitioners, the interdisciplinary health care team, community agencies, clients and families with diverse opinions, values, religious and cultural ideals.

Demonstrates knowledge of administration of total parental nutrition, enteral nutrition, chemotherapy including handling of cytotoxic drugs and biohazardous waste, and pain management therapy.
Demonstrates ability to calculate infusion rates, appropriate dosing by weight for applicable therapies and infusion pump program changes.
Demonstrates understanding and strong commitment to working in a team environment.
EDUCATION AND/OR EXPERIENCE:
RN license or eligible for licensure, in the Commonwealth of Pennsylvania required.
Minimum three years nursing experience, with minimum one year in an acute care setting, required. Home health experience preferred.
Valid Pennsylvania driver's license and willingness to do work related travel required. Completion of pharmacology exam required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in a clinical environment. This position involves potential exposure to hazardous materials. Frequently stands, walks, sits, climbs stairs and lifts/carries up to 25 lbs. May be required to drive an automobile. On-call support is required.
This position involves occupational exposure to disease and illness including, but not limited to, Hepatitis B and HIV viruses.
